Everyone is required to submit an initial assessment form. Once that intake form is received, if you qualify for our services you will have someone reach out to you to fill out our general intake packet. Everyone is required to sign our release of information form between Alt Recovery and our partner "Comfort Coaching" to receive group services. This is PDF version only.
